Camden County Partnership for Children (CCPFC) is not a treatment or social service program. We help families find, create and develop options that will support them while they work on solutions to their problems. The Child and Family Team is a crucial part of the process. The Team helps families help themselves. CCPFC helps the Team by providing the structure for the Team meetings and by helping put in place the Individual Service Plan (ISP) that the Team creates. This is called “Care Management.”
